LATE RENEWAL/EXPIRED CERTIFICATIONS OR LICENSES

The Wisconsin Department of Safety and Professional Services - Safety and Buildings Division (Department of Commerce) requires that your continuing education be completed three months prior to the expiration of your current license.  If you complete your course after the deadline or are expired, you can still renew your certification/license after you complete an approved course.  You need to contact the Department of Commerce to obtain the  “Late Credential Application Form” (Form SBD-10722).  There will be a late fee assessed in addition to the fee to renew your license.  You will need to attach a  certificate of completion of an approved course (obtained from your course provider) to the “Late Credential Application” form and mail the fee to the State of Wisconsin.   If you did not receive the late credential application form; contact the Dept. of Commerce by e-mail: madisoncred@commerce.state.wi.us or call them at 608-261-8467 to request the form.
